Eligibility for Services are those who are:
- Gainfully employed and meet the income criteria (includes dependents)
- Full-time college students
- Receiving disability income who do not have health insurance (includes spouses)
- Unemployed for less than 12 months
- Claiming child support as sole source of income
- Moving to Nelson County, Kentucky and awaiting Medicaid benefits
NOTE: Any and all exceptions are made by the Medical Director, e.g. unemployed persons with serious chronic health problems)

Treatment Ineligibility include:
- Insurance coverage
- Military identification
- Medicare, Medicaid, Worker’s Compensation
- Unemployed longer than 12 months
- Exceed the maximum income for household size per income eligibility criteria
- Treated by private physicians and referred to the Nelson County Community Clinic for prescribed diagnostic studies,
medications, or treatment
NOTE: Persons living in the household who are not related and are not dependents will not be considered to qualify or disqualify
others in the home

New Patients must bring:
- Photo identification
- Proof of residency
- Proof of household income (1040 tax form, or 2 months income documentation)

The Nelson County Community Clinic does NOT provide the following services: - Treatment to persons residing outside Nelson County
- Prescriptions written outside the clinic
- Narcotics (pain medicine)
- Physical examinations for the purpose of employment, school, athletics, workman’s compensation or disability
claims
- Financial responsibility for any medical expenses patients may incur as a result of recommended diagnostic
procedures, treatment or evaluation (e.g. X-rays, CT scans, biopsies and/or surgery)
